Desktop/Mobile Reimbursment Worksheet

Posted on January 24th, 2011 at 2:14pm

Location: Philadelphia

I-SITE is looking for an experienced PHP developer that can develop both a desktop and mobile solution for the Particle Test, a reimbursement worksheet used by sales reps. We will be testing on mobile platforms and will complete all the designs to be handed off to the developer selected for the job. This is a very simple and straightforward application with no database needs. Wireframes can be provided within the week.

The worksheet site will contain two main pages:

Guide to the Reimbursement Worksheet – This page will serve as an about page and will contain all the text that will also appear on the final printed worksheet.

Start the Reimbursement Worksheet – This begins the process of filling out the worksheet. The worksheet will be divided into the following steps:

Step 1: Complete the Doctor/Practice and Address (text input fields)

Step 2: List up to 5 Health Plan providers (text input fields)

Step 3: Next to each Health Plan listed in Step 2, click Yes or No for the Covered Benefit field (radio buttons)

Step 4: Next to each Health Plan listed, list the Lab Provider (text input fields) and then click In Network or Out of Network(radio buttons)

Step 5: Based on the results of Steps 3 and 4, display one of two options:

· If benefit is covered AND lab provider is in network, display a Green icon indicating a high likelihood of zero ($0) out-of-pocket expense for the patient

· If benefit is not covered OR lab provider is out of network, display a Yellow icon indicating that you should check with your local payer or lab partner on potential out-of-pocket expense

Step 6: User can then opt to save the results as a print-friendly PDF of the worksheet or enter one or more email addresses to have a PDF sent to.

The final PDF will contain all the information entered into the worksheet, as well as the additional information and guidelines indicated on the Guide page.

Site would need to be coded in standards-compliant HTML/CSS and optimized to run on both desktop browsers (IE, Firefox, Chrome, Safari, Opera) and on mobile devices (iPhone/iPad, Android, Blackberry). This would require a mobile detection script and either alternate CSS or alternate page builds for mobile. PHP will be used for processing the form results and determining the level of coverage at the end of the worksheet. We can provide a staging environment for build.

We can talk budget when you contact us. Thanks!
